CHSP, AGED CARE PACKAGE & Rehabilitation at home
Aged Care Package & CHSP
We provide physiotherapy home visits to residents of the Blue Mountains & Hawkesbury region under the government funded community home support program (CHSP) and aged care package's. Depending on the level of funding we can provide weekly, fortnightly or monthly home visits.
We are registered providers with most major home support programs such as Baptist Care, Just Better Care, HammondCare, Home Support Services (HSS), Medibank, St Simeon, OLOC & more.
Home visit physiotherapy under the CHSP have no out-of-pocket costs but may require a co-payment to your aged care package provider. If you wish to see our physiotherapist for a home visit either give us a call or contact your care coordinator to learn more.
Rehabilitation at Home
Hawkesbury & Blue Mountains Family Physiotherapy provides rehabilitation programs for those following operations (knee and hip replacements). We're also linked in through Rehabilitation at Home Programs - Medibank, NIB, BUPA & Home Support Services.
